Pediatric Problem Solving - Case 22

Pediatric Problem Solving – Case 22
A 1 day infant who was born by a difficult forceps delivery is alert and active. She doesn't move her left arm and keeps it internally rotated by her side with the forearm extended and pronated.

Which of the following is an expected clinical finding?

A. Intact both Moro and grasp reflex.

B. Lost both Moro and grasp reflex.

C. Intact Moro and lost grasp reflex

D. Lost Moro and intact grasp reflex.

Key points: Difficult forceps delivery. She doesn't move her left arm and keeps it internally rotated by her side with the forearm extended and pronated. All this points to Left Erb's palsy. Expected that all reflexes are lost except grasp reflex.

Right choice is: D (Lost Moro and intact grasp reflex.)
